If you are a returning patient, let us know if your personal or insurance information has changed since your last visit. 4. Co-pays, deductible amounts, and co-insurance are expected to be paid on the day of your visit. We accept most forms of payment, including check, Visa, MasterCard, American Express, and Discover cards.

If you have an out-of-pocket balance due on a bill, please make the payment in a timely fashion. 3. Call us if you have any questions or concerns about a bill or to make arrangements if you cannot pay your balance in full.
